NDIS SERVICES

ProActive Exercise Physiology is a specialist NDIS exercise physiology provider

We are an experienced disability exercise physiology provider specialising in clinical exercise interventions for people living with intellectual and physical disabilities. If you are an existing or new client you can now access our services through the NDIS.

Supports for NDIS participants

We work with our clients, carers and other medical professionals to achieve whole of life change through clinical exercise delivery. Clients can achieve improved management of disabilities, and complex and chronic conditions, such as:

 

  • Intellectual and physical disabilities, sensory impairments

  • Obesity, diabetes and arthritis, mental illness, cardiovascular and respiratory conditions 

  • Sensory impairments, learning and social disorders, ASD

  • Degenerative and age-related conditions

​Goals

Our services are based on individual assessments of condition, exercise goals, contraindications, and care requirements. Exercise physiology can directly support participants to achieve a broad range of health and lifestyle goals, for example:

  • Improved energy levels, mood, cardio capacity and overall fitness

  • Weight loss and healthy weight management

  • Improved balance, strength, coordination and mobility

  • Task specific skill development

  • Injury and fall rehabilitation and prevention

  • Greater physical independence

  • Preparing for mainstream employment and work activities

  • Participating in other sport, recreation and leisure activities

  • General health and lifestyle education

  • Improved confidence and skills for social interaction

Planning & funding

We work directly with participants, nominated planners or NDIA service provider coordinators, to understand our clients’ individual needs. We provide tailored programs to support our clients to achieve their goals. We also provide support during the planning stage, through individual assessments and communication about our services, to use in planning sessions and NDIS plan development.

You can access our services through Participant's Plan funding under the Support Category of Improved Health and Well-being. If required, amendments to a participant’s plan can be made to redirect funds into this category. Clients can choose their preferred provider to meet support needs identified in participant plans. We provide services to our NDIS clients at no out of pocket cost, charging at NDIS schedule rates.

We are mobile

We are a fully mobile provider servicing Melbourne and surrounds. We also have consultation rooms and a private purpose-built accessible exercise physiology studio in Clayton South.

Eligibility

We provide physical health and well-being support to people with a permanent disability that impacts on the management of daily activities.  NDIS clients can access group exercise physiology classes or individual sessions with our Accredited Exercise Physiologist. For non-NDIS clients, private and Medicare services are available.
